There are plenty of reasons for this to happen. Such as
- Lacking of personalize touch in your emails.
- Lack of insight into your target audience.
- Outdated email subscribers list.
- Poor Email design.
- Subject lines that makes no sense.

Nicely explained. I just wanna add one point. Good call to action button is also needed in order to increase CTR.
