Those are great basics, however, you must remember that blog outreach is something that everyone does. In order to have success doing it, you should think outside of the box. One of the best ways to do it is to do an inurl:"guest post" search in google for your particular niche. Once you are able to do that you will have a variety of blogs to choose from that you already know are accepting posts. ALLINTITLE is another way to narrow the subject of your search for niche, because google blog search will only take you so far. Another tool to use that has a 14 day free trial is ontolo.com It is a scraper that pulls the URL's of any competitor, particular subject, or any other parameters you provide. Take a look at those first, and you will have a large list to start from.
